Abstract
서론
치성 기원의 심각한 감염과 후유증은 구강악안면영역에서 이미 잘 알려진 사실이다. 이중 Ludwig’s angina는 하악치아 또는 주위 치주조직의 감염에 이차적으로 발생하여 생명을 위협하는 독성감염의 과정을 보이는 질환으로 양측성으로 설하, 이하 그리고 악하간극등을 포함하는 급성의 봉와직염이다. 임상 증상으로는 심한 구강저의 부종과 갈색의 악하부위 조직의 경결감과 혀의 거상 및 후방변위의 소견을 보이고 질환의 진행에 의해 인두와 하인두의 후측방변위를 일으켜 기도폐쇄를 일으켜 사망에까지 이르는 치명적인 질환이다. 이질환은 오래전부터 알려져 왔지만 1836년 Frederick von Ludwig에 의해서 정식으로 명명되었으며 1939년 Grodinsky에 의하여 진단을 위한 기준이 정하여 졌다. 이와 같은 구강영역의 감염은 최근의 치과치료와 항생제의 조기 투여로 인하여 임상적으로 매우 드문 질환이 되었지만 이 질환이 발생한다면 여전히 응급 상황으로 간주되어지고 있다. 또한 항생제를 사용하기 이전에는 치사율이 50%이상이었으나 페니실린의 이용으로 4%로 치사율의 급격한 저하를보이고 있는 추세이다.
구강악안면영역의 감염은 숙주의 방어기전에 이상을 초래하는 Cushing’s syndrome과 같은 전신질환과 연관된 경우에는 그심도와 안면간극을 통한 전이속도가 더욱 심각한 양상을 나타낼 수 있다. Cushing’s syndrome은 부신피질 기능장애로 ACTH 또는 글루코코티코이드를 만성적으로 장기간 투약하였을 경우 나타나는 의원성 질환이다. 이와같은 약물을 장기간 투여한 환자에서 세균 및 진균의 병원균에 대한 숙주의 방어기전의 현저한 저하를 보이며 감염에 대한 이환률도 증가하게 된다. 스테로이드를 장기간 투여한 환자에서 감염의 심도도 점점 증가하게 된다. 이에 구강악안면 영역에서도 스테로이드 장기투여로 인한 부신피질 기능장애를 가진 환자를 접할 기회가 증가하고 있고 대부분의 환자 자신이 이환여부를 인지하지 못하고 있으며 이러한 약물을 약국에서 쉽게 구할 수있기 때문체 이러한 환자의 병력조사 및 전신적인 평가에 대한 세심한 주위가 필요하다고 하겠다. 이에 본 교실에서는 Cushing’s syndrome 있는 Ludwig’s angina 환자에서 기관내 삽관술, 스테로이드 치료, 항생제 투여 및 수액요법, 절개 및 배농등의 처치를 농하여 다소의 지견을 얻었기에 문헌 고찰과 함께 보고하는 바이다.
Ludwig’s angina is a serous and potentially life threatening infectious disease involving
all of the submental, submandibular, and sublingual space bilaterally. It is characterized
by severe swelling and broad-like induration of the tissues in the submandibular region
and elevation of the tongue and flood of mouth with partial immobilization of these
structures.
A common cause of Cushing’s disease is an iatrogenic problem related to chronic
administration of either ACTH or glucocorticoids. Patients who administrated
glucocorticoids altered responese infection and wound healing. More important, many
patients who are taking or have been taken glucocorticoids have risk of developing
acute adrenal insuffciency (adrenal crisis) when place in stressful situation.
The treatment of infections in Cushing’s syndrome are reduction of number of
microbes through the use of appropriated antimicrobial agents and proper surgical
drainage and appropriated airway management and prorer steroids therapy that increased
administration of glucocorticoids.
In this paper, we report a rare case of Ludwig’s angina with Cushing’s syndrome. A
45-years old women was referred to our clinic in November 1996 for treatment of
infetious disease. Clinical examination of this patient showed a painful swelling of
submental space, elevation of tongue and difficulty of respiration or ’Moon Face’,
’Bufallo hump’, striae of abdomen and ecchymosis of extrmities.
We treated through the quickly airway management using blind oroendotracheal
intubation, antimicrobial injection of large quantity, increased adminstration of
glucocorticoids and proper surgical drainage. And We aquired satisfactory result.
